National Patient Safety Agency
The NPSA is a Special Health Authority created in July 2001 and it describes its role as 'to co-ordinate the efforts of the entire country to report, and more importantly to learn from mistakes and problems that affect patient safety'. Its website gives information about alerts and advice and links to its publications.
